Revatio use:
Revatio is prescribed in the dose schedule of three tablets of 20 mg per day to be taken with an interval of 5 to 6 hours in between each dose along with food or without food at the convenience of the patient.
In various studies conducted there were no better results by increasing the dose of the drug.
Missed dose:
Great care should be taken so that the dose of Revatio is not missed, in case a dose gets missed and it is time for the next dose, proceed with the next dose and maintain as scheduled. The dose of 3 tablets per day should not be exceeded.
Revatio storage:
No special storage conditions are specified store at room temperature away from the reach of children.
Revatio possible side-effects:
If a person on administration of Revatio develops symptoms of edema in the lungs, presence of Pulmonary veno-occclusive disorder may be considered as the cause.
Since Revatio has vaso-dilation effect, it can affect cardiovascular status of the person having of veno-occclusive disorder. There is no data confirming use of Revatio in such patients.
There is absence of clinical data in respect of controlled study in respect of safe use or benefits of using Revatio in following conditions, necessary caution should be observed while advising Revatio therapy:
- Patients having history of myocardial infarction, stroke, or arrhythmia attacks during the previous 6 months.
- Patients suffering from coronary artery disorders leading to angina not stabilized.
- Patients suffering from high blood pressure > 170/110.
- Patient presently continuing treatment with Bosentan.
PDE5 inhibiting agents like sildenafil, alpha-adrenergic blocking drugs act as vasodilators causing lowering of blood pressure. Use of vasodilators in combination, have effects of causing symptomatic hypotension. The fact should be kept in mind. None of case of syncope or fainting feeling was observed in the interaction studies.
The safety of use in combination of PDE5 inhibitors and alpha-blockers need to be considered in respect of other variables like fall in intravascular volume and use of anti-hypertensive drugs along with Revatio.
Revatio warnings and precautions:
There are certain contraindicated conditions like:
- Patients experiencing hypotension on resting.
- Patients having fluid depletion.
- Patients suffering from severe left ventricular outgoing hindrance.
- Patients having deficiency in Autonomic dysfunction.
In above conditions use of a vasodilator in a great measure can further deteriorate the condition of the patient with presence of veno-occlusive disease.
Use of Revatio in combination with any of organic compound having Nitrate in its configuration is contraindicated. Nitric oxide liberated affects the cGMP pathway. Sildenafil active ingredient of Revatio in combination with organic nitrates causes increase in its potency in respect of hypotensive action.
Hypersensitivity Reaction:
REVATIO is not recommended for use in patients having hypersensitivity to sildenafil or any component of the tablet.
Rarely conditions of hypersensitivity were observed in on use of Sildenafil like anaphylactic shock.
Overdose:
It is advised that no attempt should be made to increase the prescribed overdose, if by mistake overdose has happened and patient feels unmanageable he should be taken to a specialized hospital for the management of the conditions presented.
Effects on Bleeding:
An incidence of epistaxis in 13% patients using sildenafil suffering from PAH along with connective tissue disease (CTD). This was not observed in patients of primary pulmonary hypertension (PPH).
Incidence of epistaxis was a bit higher in patients treated with sildenafil along with concomitant oral vitamin K antagonist.
The safety of REVATIO is unknown in patients with bleeding disorders or active peptic ulceration.
Use in combination with Ritonavir and CYP3A Inhibitors:
The use of Revation with protease inhibitor Ritonavir, which is a highly potent CYP3A inhibitor considerably increases serum concentration of sildenafil. Considering this use of Ritonavir or CYP3A inhibitors with REVATIO is not advised.
Effects on the Eye
The patient needs to be forewarned to get immediate medical care if there is abrupt loss of vision in one or both of the eyes by the use of PDE5 inhibiting agents including REVATIO. Such a happening possibly is a sign of non-arteritic anterior ischemic optic neuropathy, which is cause of impairment or loss of vision.
Hearing Impairment:
The patients be advised to access immediate medical care if one encounters sudden impairment or loss of hearing with the use of PDE5 inhibiting agents
i.e. Revatio. Such condition is preceded by tinnitus and dizziness.
Use of Revatio along with PDE5 inhibitors:
Sildenafil is also available as Viagra. The safe and effective use of Revatio with Viagra or other PDE5 inhibiting agents are not studied. Patients on Revatio treatment should not use Viagra or other PDE5 inhibiting agents.
Prolonged Erection:
It is suggested to use Revatio carefully in patients with structural deformity of penis like angulation, cavernosal fibrosis, or Peyronie’s
Disease or in patients who are predisposed to priapism, sickle cell anemia, multiple myeloma, or leukemia.
If there is erection of penis persisting for more than 4 hours, patient needs urgently medical help. If painful erection is for more than 6 hours with no treatment given it can cause damage to penis tissues with permanent impotency.
